AAIU Reports

The following is a listing of Investigation Reports published by the Air Accident Investigation Unit concerning accidents and incidents that occurred within Ireland, including its airspace and territorial waters. This list will also include Investigations concerning Irish-registered and/or operated aircraft which were delegated to the AAIU by Foreign Accident Investigation Authorities in accordance with ICAO Annex 13. Incident: Powered para-glider, Salterstown Co. Louth, 26 July 2009: Report No 2010-011

Synopsis

After a flight of one hour, the Pilot decided to practise a forced landing exercise. He overshot the landing point and struck power lines near the field boundary. The para-glider suffered minor damage but there was a loss of power to a significant number of electricity users as a result of the incident.

SYNOPSIS


The glider was carrying out a closed-circuit cross-country flight from Birr Airfield, Co. Offaly (EIBR). Due to deterioration in the weather conditions, the Pilot decided to make an unscheduled out-landing in a field near Durrow, Co. Laois. The glider landed on a down-slope, and the Pilot ground looped to the right to avoid impacting the field boundary fence and hedge. The fuselage sustained significant damage during the ground loop manoeuvre.

SYNOPSIS

The helicopter departed Weston Airport (EIWT) at 16.02 hrs on a training flight with an Instructor and Student on board.  The Instructor stated to Dublin Air Traffic Control (ATC) that his intention was to operate south of Enfield between 1,000 and 2,000 ft.  The last communication with Dublin ATC was at 16.12 hrs as the helicopter transited into Class G airspace[1] outside the Dublin Control Zone (CTR).  Following this exchange, no further communication was made.  During a probable low-level transition after a practice autorotation (Section 1.18.1 Autorotation), the helicopter impacted disused electrical power lines at Kilshanchoe, Co. Kildare. An alert was raised following enquires made by family members the following morning.  The wreckage of the helicopter was located the following morning by an Irish Coast Guard helicopter.  The accident was not survivable and both occupants were fatally injured.  There was no fire.

[1] Class G airspace:All uncontrolled airspace within the Shannon FIR except for those areas designated as Class A and Class C.

SYNOPSIS _x000D_

The aircraft had departed Dublin and was climbing towards cruising altitude when the Purser reported to the Captain that another Cabin Crew Member (CCM) appeared unwell and that passengers appeared to have fallen asleep quickly after take-off. Following a discussion the flight crew went on oxygen, declared an emergency and returned to Dublin where the aircraft landed without further incident. No defect was subsequently found on the aircraft. The report of the Investigation makes three safety recommendations.

SYNOPSIS

On climb out, at between 100 ft and 150 ft above the ground, the engine stopped.  The aircraft made a forced landing into an agricultural field adjacent to the airfield.  There were no injuries.  The aircraft suffered substantial damage.  The Investigation found that the accident was probably due to carburettor icing.  The Report makes three safety recommendations.

The Occurrence

On the afternoon of 27 June 2010, a Boeing 737-800 parked on stand 109R at Dublin Airport, following a flight from Krakow.  During disembarkation from the rear door the first three passengers emerged from the aircraft onto the mobile air stairs unit.  The stairs then partially collapsed trapping and causing serious injury to a passenger’s leg.

SYNOPSIS

 


The glider became low on the approach to Gowran Grange Airfield, struck electricity supply cables, stalled and impacted nose first into a ploughed field adjacent to the intended landing area.  The glider suffered significant damage.  The Pilot evacuated the glider unaided and reported receiving only minor scratch injuries.

SYNOPSIS

During circuit flying, just before levelling from a descent, the left engine stopped. The Pilot carried out a forced landing into a field adjacent to the airfield. The aircraft suffered substantial damage. The engine stoppage was probably due to carburettor icing.  The Pilot was not injured.

Serious Incident: Boeing 757-28A, C-GTBB, Dublin Airport, 29 May 2009: Report No 2010-002

Synopsis

The aircraft landed on Runway (RWY) 10 in EIDW in conditions of poor visibility. On roll-out, it passed a ride-on grass mower which was travelling eastwards along the runway approximately 18.5 metres (m) to the right of the centre-line. The driver of the mower was unaware that an aircraft was arriving until he heard it on the runway behind him. Prior to the incident, ATC was informed that all grass-cutting equipment had vacated the field.

The Report of the Investigation makes one Safety Recommendation.
